Payment Information

Pure Physio accepts most insurance plans. We are in-network with Medicare. With commercial insurance companies (for example: Horizon BCBS, Aetna, Cigna, UnitedHealthcare, etc) we have chosen to participate as an out-of-network provider. With this model, we are not bound by insurance companies’ stipulations and limitations. This allows us to provide longer one-on-one sessions and give all necessary treatment without restrictions. We will verify your insurance benefits and review it with you prior to your appointment. This is not a guarantee of your coverage as each patient is ultimately responsible for knowing the details of their own insurance policy but we can help you understand them better.

 

Private pay option is also available. Payment for treatment is expected at the time of treatment. Private pay clients may choose to submit a superbill to their insurance provider for reimbursement. Pure Physio cannot guarantee full or partial reimbursement from your provider as each policy is unique.

Do you accept medicare? 

Yes, we are an in-network medicare provider.

Why do you choose to remain out-of-network with most health insurances? 

Pure Physio is an out-of-network provider with commercial insurance companies. We prefer to treat patients with their individual goals in mind without any limitations set by insurance companies. We choose to work for our patients. This model allows us to provide one-on-one high quality treatment you need. We can also provide our private pay clients with a superbill for potential reimbursement from their insurance company.

What is a superbill? ​

A superbill is a receipt of services provided. It will include details about Pure Physio as a provider, diagnosis (ICD-10 codes), details regarding treatment charges (CPT codes), and the date of service. Patients can then submit this superbill with a claims form to get directly reimbursed from their insurance company if eligible. We cannot guarantee if partial or full reimbursement will be provided since each plan is unique.

 

We perform all other in-network and out-of-network billing for those who choose to use their insurance plans for treatment.
